Services include connection to mental health or substance use disorder treatment, DSHS benefits and SSI applications, employment, probation completion, maintaining sobriety, and helping to obtain and maintain stable housing.
eligibility
Transition House is open to referrals only. To be eligible for referral, you must:
- Be homeless at entry
- Have a documented mental health or substance use disorder diagnosis
- Be exiting jail, detox or inpatient treatment
- Be at least 18 years old
- Have past history of residency in Skagit County
- Pay a service fee of 30% of your income, or zero with no income
- Be committed to compliance with case management and individual case management plans for more permanent housing
- Be committed to sobriety and agree to monitoring through urinalysis and breathalyzers

- Mental Health Treatment Verification
- Substance Use Disorder Treatment Verification
* All referents will be notified via email about pending openings within the program.
Transition House if provided in collaboration with Skagit County Public Health and Community Services.
